The Philippine Consulate General in Barcelona performs civil registry services for Filipino citizens residing in Spain and Andorra.  These include reports of birth, marriage and death.



Civil Registration

Civil registration is a continuous, permanent and compulsory recording of vital events occurring in the life an individual such as birth, marriage, and death, as well as court decrees, and legal instruments affecting his/her civil status in appropriate registers as mandated by Republic Act 3753 or the Civil Registry Law. 



Uses of Civil Registry Documents

  1. To establish the occurrence of birth, marriage or death, and therefore provide prima facie evidence of facts surrounding these events

  2. To serve as identification in legal and personal transactions such as applying for jobs, obtaining passports, applying for entrance into schools, claiming insurance benefits, etc.

  3. To provide data on the number of births, marriages, deaths, fetal deaths, adoptions, and the like, which in turn are essential in a country’s development planning

REPORT OF BIRTH

These requirments are for children born in Spain of Filipino parent/s


Basic Requirements


The Report of Birth and Passport Application Form.pdf forms must be accomplished by either of the Filipino parents or the legal guardian.  The child must be brought to the embassy.


  1. Duly completed Report of Birth

  2. Original and five (5) photocopies of the following documents:

    1. Birth Certificate issued by the Spanish Civil Registry (Certificado Literal de Nacimiento)

    2. Passports of parents

    3. Marriage contract of parents (if applicable)

  3. Cash payment for application and processing


Additional Requirements (for new-born babies whose parents are not legally married and/or Delayed Registration)

 

  1. Affidavit of Acknowledgement of Paternity to be executed by the father

  2. Affidavit by the father consenting to the use of his surname

  3. Affidavit of Delayed Registration (if more than one year old)

  4. In case of Delayed Registration, Negative Certification of Birth issued by Philippine Stadistics Authority (PSA) and four (4) photocopies

  5. Cash payment for processing


The Consulate will forward the Report of Birth to the National Statistics Office (NSO) in the Philippines.  Applicants may retrieve the NSO-certified birth certificate in the Philippines a minimum of three (3) months after reporting the birth to the Embassy.

REPORT OF DEATH

Basic Requirements

  1. Duly accomplished Death Certificate form in four (4) originals

  2. Original death certificate (Literal de Defunción issued by Registro Civil) and four (4) photocopies

  3. Original Medical Death Certificate and four (4) photocopies

  4. Four (4) photocopies of deceased passport and/or Oath of Allegiance

  5. Cash payment for processing  

The Embassy will forward the Report of Death to the National Statistics Office (NSO) in the Philippines.  Applicants may retrieve the NSO-certified death certificate in the Philippines a minimum of three (3) months after reporting the death to the Embassy.
