Acknowledgement of Documents


Requirements

 

  1. Original and duplicate (or photocopy) of the document/s to be acknowledged.  The Embassy will retain the duplicate.

  2. Personal appearance of the executor / signatory of the document.  The original document must be signed in the presence of the consular officer / staff.

  3. Photocopy of the executor / signatory’s valid identification which bears his/her photograph and signature (passport, driver’s license, voter’s Id, SSS Id, etc.) and/or Oath of Allegiance (if Dual Citizen)

  4. Cash payment for processing fee of each document


The acknowledged or notarized documents signed by the consular officer may be collected after three (3) working days.


Affidavits, Sworn Statements, Certification, etc.


-    Personal appearance of the person/s executing/signing the document

-    Original documents and three (3) photocopies

-    Philippine passport/Spanish passport Oath of Allegiance (if Dual Citizen)

-    Tarjeta de Residencia (Residence Permit)

-    Cash payment for processing fee of each document



Affidavit of Support and Consent / Travel Authorization (for minors with one or both parents in Spain)


  1. -   Personal appearance of parent/s

  2. -   Original documents and two (2) photocopies of the following:
           - NSO - authenticated Birth Certificate of Child or Certificado Literal de Nacimiento
           - NSO Marriage contract or Certificado Literal de Matrimonio of parents, if married
           - Philippine passport/Spanish passport and/or Oath of Allegiance (if Dual Citizen)
           - Tarjeta de Residencia (Residence Permit)

        - Passport of travelling companion
